This service is designed to support individuals with complex care needs, chronic health conditions, disabilities, or those who require rehabilitation support following hospital discharge. The live-in care model ensures the presence of dedicated care staff who will reside with the service user for agreed periods, typically ranging from several days to weeks, with appropriate handover arrangements in place to ensure seamless service continuity. Live-in carers will support with all aspects of daily living, including personal care, meal preparation, household tasks, companionship, and emotional support.
